The types of personal health information I collect includes your name, date of birth, address, email address, phone number, health history, and records of the care provided to you. I will provide your personal health information to a third party only with your signed consent. The original Health History Form is stored in a safe box under a lock, with me being the only person who has access to it. A scanned copy of the Health History Form is stored in an electronic format on a hard drive of my personal computer and is protected by a strong randomly generated password, which effectively means that no one by myself can access it. The information is stored for a period of at least 10 years after your last visit.
You have the right to have access to your health records that I keep and ask me to correct a record if you believe it is inaccurate or incomplete. Please contact me for more information.
